Benefit Pathways Volunteer Agreement

This Volunteer Agreement ("Agreement") is entered into between Benefit Pathways (the "Program") and the undersigned individual (the "Volunteer"). By enrolling in the Benefit Pathways program and participating in volunteer activities, the Volunteer agrees to the following terms and conditions:

1. Purpose of the Program

Benefit Pathways provides structured volunteer opportunities, documentation tools, supervision, and case management support to assist members in meeting applicable work or community engagement requirements tied to public benefits (including but not limited to SNAP, Medicaid, SSI/SSDI, or other assistance programs). Participation does not guarantee continuation, approval, or eligibility for any public benefit.

2. Voluntary Participation

The Volunteer acknowledges that:

  • Participation is voluntary and not an employment relationship.

  • The Volunteer is not entitled to wages, salaries, or employee benefits.

  • Volunteer activities are intended for skill-building, community contribution, and compliance documentation purposes only.

3. Eligibility & Enrollment

The Volunteer confirms that:

  • They have voluntarily enrolled in Benefit Pathways.

  • They are responsible for determining whether the program aligns with their personal benefit requirements.

  • Final enrollment occurs after orientation and informed consent.

4. Volunteer Activities

The Volunteer agrees to:

  • Participate only in approved volunteer activities assigned or authorized by Benefit Pathways.

  • Accurately record volunteer hours, activities, and required documentation using the Benefit Pathways platform or approved reporting tools.

  • Perform duties responsibly, ethically, and in accordance with community standards.

5. Time Commitment & Reporting

  • Volunteers are responsible for completing their required monthly hours as applicable to their benefit program.

  • All hours must be truthful, verifiable, and submitted on time.

  • Falsification or misrepresentation of hours or activities may result in removal from the program.

6. Supervision & Compliance Support

Benefit Pathways agrees to:

  • Provide supervision, guidance, and documentation support.

  • Monitor participation and provide feedback when concerns arise.

  • Offer advocacy support when appropriate; however, final decisions rest with government agencies.

7. Conduct & Professional Standards

The Volunteer agrees to:

  • Treat staff, fellow volunteers, and community members with respect.

  • Maintain appropriate behavior, confidentiality, and boundaries.

  • Refrain from harassment, discrimination, fraud, or unsafe conduct.

Violation of these standards may result in immediate termination from the program.

8. Confidentiality

The Volunteer understands that:

  • They may have access to sensitive or personal information.

  • All such information must remain confidential and may not be shared without authorization.

9. Technology & Platform Use

  • The Volunteer agrees to use the Benefit Pathways platform responsibly.

  • Login credentials must not be shared.

  • Data usage is required to facilitate tracking, supervision, and compliance support.

10. Transportation & Expenses

  • Volunteers are responsible for their own transportation and incidental expenses unless otherwise stated.

  • Benefit Pathways is not obligated to reimburse costs incurred during volunteer activities.

11. No Guarantee of Benefits

The Volunteer acknowledges that:

  • Benefit Pathways does not guarantee approval, continuation, or reinstatement of any public benefits.

  • Compliance determinations are made solely by the relevant government agency.

12. Termination of Participation

  • The Volunteer may withdraw at any time by providing notice.

  • Benefit Pathways may suspend or terminate participation for noncompliance, misconduct, or failure to meet reporting requirements.

13. Release of Liability

To the fullest extent permitted by law, the Volunteer releases Benefit Pathways from liability for injuries, losses, or damages arising from participation, except in cases of gross negligence or willful misconduct.

14. Acknowledgment & Agreement

By enrolling and participating, the Volunteer acknowledges that they have read, understood, and agreed to the terms of this Agreement.
